Transforming recovery

We’re building a community of survivors

Heart disease and stroke are the two leading causes of hospitalization in Canada — accounting for a staggering 350,000 hospitalizations every year. They can happen without warning and tear families apart. You probably know someone who’s been affected

About 3.5 million Canadians are living with the effects of heart disease and stroke every single day. Survivors and their families need support and information to help them manage their condition, navigate the healthcare system and improve their quality of life.

Improving patient recovery pays big health dividends. Participating in a cardiac rehabilitation program after a heart attack, for example, lowers subsequent death rates by 50%. Managing your risk factors after heart disease or stroke is vital to preventing further incidents.

That’s why we’re committed to helping survivors and their care partners achieve the best recovery and quality of life possible and prevent future events. See our latest results.

Our action plan:
  • Partner with survivors and their care partners, supporting and connecting them to achieve the best recovery and quality of life possible and prevent future events.
  • Reach at least 50% of stroke survivors and 35% of heart disease survivors with our recovery resources.
  • Fund world-leading research on stroke recovery through the Heart & Stroke Foundation Canadian Partnership for Stroke Recovery.
  • Support survivors through programs including Living With Stroke.
