  • Riverview Health Centre Neurology Rehabilitation Stroke Program: This program provides comprehensive, interdisciplinary care for adults with neurological impairments due to a stroke.
  • College of Physiotherapists of Manitoba: CPM is a self-supported professional body that regulates the practice of physiotherapy in Manitoba. They offer a directory of physiotherapists in Manitoba.
  • Geriatric Program Assessment Team (GPAT): This program assesses frail people over the age of 65 years, living in Winnipeg.. Clients are seen by a geriatric clinician in their own homes or a hospital for an assessment.
  • Get Better Together: A workshop designed to help Manitobans with ongoing health conditions take control of their health. This program is ran by volunteers who also live with health issues and understand the challenges of managing symptoms, medications and health care needs.   • TeleCARE/TéléSOINS Manitoba: A free phone based program offering education, support, health monitoring and health behaviour coaching. Registered nurses and dietitians work with you to create an individualized program and provide tools to help self-manage heart failure. 204-788-8688 or toll free 1-866-204-3737.
  • Cardiac Health Foundation of Canada: Cardiac rehabilitation is offered across Manitoba as a standard of care for those living with heart disease. This includes a medically supervised 16 week education and exercise program designed to help individuals develop the skills, capacity and confidence to lead a healthier life. Spouses and partners are encourage to attend the educational sessions as well.
